A father was horrified to find his family dead after returning home from work.
The mother stabbed her two young daughters to death because she thought that her older daughter had cancer, an inquiry heard.
Marta Galikowska , 27, of Staffordshire, England, also died when she turned the knife on herself and cut her own throat.
The bodies of Marta, her 5-year-old daughter Maja and 18-month-old Olga, were discovered by her husband Marcin Galikowski, 28, when he returned from work.
The North Staffordshire Coroner’s Court heard that the mother was convinced that Maja had cancer after she found a lump under her jaw.
Ms. Galikowska was prescribed antidepressants by a doctor a week before her death after telling her husband she wanted to kill her children and commit suicide.
The case was referred to social services, but after visiting the family, social workers said that they had no immediate concerns about the safety of the children.
After the deaths, the police arrested Mr. Galikowski, but he was released without being charged.
Tests revealed that the lump under Maja’s jaw was a benign cyst.
