(Scroll down for video) A grandmother had died while heading home from church after her two grandchildren played with a loaded gun, according to police in Kentucky.

The Pulaski County Sheriff’s Office said that they launched an investigation after 68-year-old Eutta Phelps was shot in the back of the head.

According to the police investigation, the incident unfolded on Wednesday night.

Phelps and her two grandchildren, aged 12 and 17, went to church.

While they were driving home, the two grandchildren were in the back seat of the car while Phelps was in the front passenger seat.

The two grandchildren were passing a loaded gun to each other when it fired a shot, at around 9:00 p.m.

The bullet hit Phelps in the back of the head.

Paramedics who arrived at the scene on Highway 1003, took Phelps to the Lake Cumberland Regional Hospital, where she died.

Police do not plan to file charges in this case because they believe this was an accidental shooting.
